For this reason, we offer the Wellness Benefit to our policyholders. Aflac New York will pay a benefit for routine examinations or other preventative tests.

Your benefit period and type of screenings covered may vary by type of policy. Covered screenings may include:

• annual physical examinations
• mammograms
• pap smears
• eye examinations
• immunizations
• flexible sigmoidoscopies
• prostate-specific antigen (PSA) tests
• ultrasounds
• blood screenings

Services must incur for a charge and be supervised or recommended by a physician (this benefit may not be available with all products).
