Life Claims
To file a life claim or to request a life claim packet, call us toll-free at 1-800-366-3436. We will mail letters to the beneficiary requesting the following information:
- A certified death certificate (required for all death claims)
- A toxicology report on all accidental-death claims (when applicable)
- An Authorization signed and dated by the next of kin
